I don't doubt this explanation at all.  It shows a major defect in the 
corporate culture which needs to be addressed, and until it is, the 
trouble will continue.

It reminds me of recent information regarding some car issues with 
Mitsubishi, where the lower and middle managers and front end customer 
service workers were "protecting" management from the huge number of 
failures and complaints, and safety issues.  At the end of the day, the 
CEO resigned over this, although he may have been "the last to know".
