Application Steps

  1. Fill out the PDF application below and gather the required documentation.
  2. Email a completed copy of application and CV to Carl Kriigel, Forensic Video Certification Board Secretary. The email address can be accessed using the button on the application. Indicate in your email whether you are planning on testing individually (with a proctor that meets the guidelines), or at one of the upcoming group locations.
  3. Mail your signed, completed application to the address listed on the application. Be sure to include a copy of your CV and all other relevent documentation. If you did not complete the credit card payment information section of the application, be sure to enclose payment as instructed in the application.
