I've been rereading some of the posts a week or two back about brief model
releases for street photography.

While I'm not a lawyer, I face similar issues in another field (I 
specialise
in recording traditional music in improbable places) and am accustomed to
concocting releases that are brief and clear while covering the essential
bases.

After re-arranging the elements of Arthur's street version, here's what I
came up with:

For value received of ________,  ________________ assigns to the
photographer, NAME, all rights to photograph(s) taken on _______ at
________, and agrees that these image(s) is/are the property of the
photographer who may at his discretion use it/them for any purpose and in
any way.
Name of adult, child, or pet, photographed _________________
Accepted: ____________________ date ____________
The person signing this release is of legal age to sign a contract (or can
do so on behalf of a minor or an animal/pet).
